Rate how likely the harm is to occur, given the controls already in place, on a scale of 1 to 5: 1 rare, could happen only in exceptional circumstances; 2 unlikely; 3 possible, could happen at some time; 4 likely, will probably happen at some point; 5 almost certain, expected to happen in most circumstances or happens regularly.
Rate the worst realistic outcome on a scale of 1 to 5: 1 negligible, minor first aid; 2 minor, injury needing treatment with short absence; 3 moderate, injury causing more than three days absence, reportable to the HSA; 4 major, serious injury or long term disability; 5 catastrophic, a fatality or multiple serious injuries.
Multiply likelihood by severity for a score from 1 to 25. Typical bands: 1 to 4 low, acceptable, monitor; 5 to 12 medium, reduce within a set timescale; 15 to 25 high, act immediately and stop the activity if necessary. Reassess after the new controls to show the residual risk. The bands and timescales should be written into the safety statement so everyone uses them the same way.
Scoring the likelihood as if no controls existed, which makes everything high. Scoring severity as the most common outcome instead of the worst realistic one, which makes fatal hazards look minor. Treating the score as the end of the job; the matrix prioritises, the controls protect. And spending an hour on numbers for a hazard whose control is obvious: fix it and move on.
