Nobody may enter a confined space to carry out work unless it is not reasonably practicable to do the work without entering. Cleaning from outside, remote cameras, modified processes and better access designed in from the start are all expected to be considered first. Where entry cannot be avoided, the employer must ensure a safe system of work is in place and followed, based on a risk assessment carried out by a competent person.
The Code of Practice describes what the safe system covers: isolation of the space from pipes and power; cleaning and purging; testing and monitoring the atmosphere before and during entry; ventilation; respiratory protective equipment where the atmosphere cannot be made safe; suitable lighting and tools, intrinsically safe where flammable atmospheres are possible; communication between the entrant and the attendant; limits on working time; and a permit to work that records the checks and authorises the entry.
Nobody may enter or work in a confined space unless suitable and sufficient arrangements for rescue have been made. That means a rescue plan specific to the space, trained rescuers with the equipment to retrieve a casualty without entering where possible, breathing apparatus, first aid and resuscitation equipment, and arrangements to summon the emergency services. Rescue equipment must be maintained and rescuers must practise.
Everyone involved must be competent for their role: the person assessing the risk, the person issuing the permit, the entrants, the attendant at the entrance and the rescue team. Training covers hazards and specified risks, gas testing, respiratory protection, the permit system, communication and emergency procedures, and is refreshed regularly with practical rescue exercises.
