Suspect anaphylaxis when a sudden illness involves the airway, breathing or circulation, usually with skin changes: swelling of the lips, tongue or throat, a hoarse voice or difficulty swallowing; wheeze, persistent cough or struggling to breathe; pale, clammy skin, a fast weak pulse, dizziness or collapse; widespread hives, flushing or itching. Symptoms typically appear within minutes of exposure. A known allergy plus any breathing or circulation sign is anaphylaxis until proven otherwise.
Call 112 or 999 and say anaphylaxis. Use the casualty's own auto injector if they have one, or the emergency device where the workplace holds one; the adult dose is 300 micrograms, the child dose for 15 to 30 kg is 150 micrograms. Remove the safety cap, press the tip firmly into the outer thigh, through clothing if necessary, and hold for the time the device specifies, three seconds for EpiPen, ten seconds for Jext. Note the time. If there is no improvement after five minutes, give a second injector if one is available. Never inject into a vein, buttock, or through a pocket with objects in it.
Lie the person flat with legs raised if they feel faint or their circulation is failing; sit them up if breathing is the main problem; place a pregnant woman on her left side. Do not let them stand or walk, even if they say they feel better; sudden standing after anaphylaxis has caused cardiac arrest. Everyone treated for anaphylaxis goes to hospital by ambulance, because a second wave reaction can occur hours later. Hand the used injector to the ambulance crew.
